چکیده

OBJECTIVE To compare the effect of donor-recipient trephine-size disparity on spherical equivalent, and visual outcomes in penetrating keratoplasty for keratoconus. METHODS A prospective randomized clinical study conducted between April 2000 and April 2004 in the Ophthalmology Department, Shaheed Sadoughi Hospital, Yazd, Iran. Forty patients with keratoconus were randomly assigned to 2 groups. In group I (n=20) the patients were operated on with under-sized blade (0.25 mm disparity[D]), and in group II (n=20) with over-sized (0.50 mm D) trephine. RESULTS The spherical equivalent in group I was -2.61 +/- 2.81D, and -3.92 +/- 3.21D in group II. We found a better final visual acuity in group I compared with group II. More myopic shift was observed in group II, with greater disparity. Best-corrected visual acuity (BCVA) was better at the final follow-up in group I, compared with group II. Fourteen patients (70%) achieved BCVA 20/40 or better in group I, whereas in group II only 10 patients (5.0%) achieved this vision. Mean keratometry was 44.35D in group I, and 45.05D in group II. CONCLUSION We conclude that using donor-recipient under-sized trephine (0.25mm D) can be considered a reliable and effective method in reducing postoperative myopia.

Penetrating Keratoplasty for Keratoconus – Excimer Versus Femtosecond Laser Trephination

BACKGROUND In case of keratoconus, rigid gas-permeable contact lenses as the correction method of first choice allow for a good visual acuity for quite some time. In a severe stage of the disease with major cone-shaped protrusion of the cornea, even specially designed keratoconus contact lenses are no more tolerated. In case of existing contraindications for intrastromal ring segments, corneal ...

Visual and Refractive Results of Cataract Surgery by Phacoemulsification Method in Patients with Keratoconus

Purpose: To examine the visual and refractive results of cataract surgery by phacoemulsification method in patients with keratoconus. Methods: In this retrospective study, the files of patients with keratoconus, who underwent phacoemulsification between 2008 and 2017, were evaluated. The intraocular lens power calculation formula was SRK/T. Post-operative visual and refractive results were anal...

Myopia following penetrating keratoplasty for keratoconus.

The frequent occurrence of spherical myopia after penetrating keratoplasty for keratoconus is partly the result of the excessive dioptric power of the grafted cornea which occurs when the diameter selected for the donor button is greater than the diameter of the host incision. This excessive power could be reduced by eliminating disparity between the diameters of the graft and host. To determin...

Outcomes from a modified microkeratome-assisted lamellar keratoplasty for keratoconus.

To improve visual and refractive outcomes, microkeratome-assisted lamellar keratoplasty for the treatment of keratoconus (exchange of a 9.0-mm anterior recipient lamella with a 9.0-mm donor lamella, using a 200-μm head for the former and a 300-μm head for the latter) was modified by adding a 6.5-mm incomplete full-thickness incision in the recipient bed before suturing the donor graft in place....
